I understand that providing false or incomplete information may result in the refusal of this application or termination of camp participation without a refund. I acknowledge that I have read, understood, and agree to abide by the camp's terms and conditions, including but not limited to the participation requirements, payment obligations, and cancellation policies. I am aware that cancellations must be made at least 7 days prior to the camp start date to qualify for a full refund, minus any processing fees. Cancellations made after this period may not be eligible for a refund. Additionally, I understand that participation in the camp is voluntary, at my own risk and risk of my child and as a consideration for the right to participate I hereby, for myself, my heirs, executors, administrators or personal representatives waive any and all rights, claims or causes of action of any kind arising out of participation in this camp, thereby releasing from liability the camp organizers, staff, coaches, affiliates, and partners. I grant permission for the Camp staff to seek medical treatment for my child in case of injury, accident, or illness during the camp period if I am not immediately available to give consent. I agree to be responsible for any medical or other charges in connection with my child's attendance at the camp.
